D285473 titled “Golf Club Head,” which issued on September 2, 1986 and was assigned to Orizaba Golf.

Apparently the design was a little ahead of its time, as this article indicates that the company had to replace more than $1.5 million worth of the clubs due to a variety of club head failures.
